In memory care, assisted living and skilled nursing, you will often hear the word “readmission,” which is when a patient or resident is readmitted to a hospital shortly after being discharged. While some readmissions are unavoidable, many are preventable with the right practices. Silverado communities have industry-leading readmission rates, so we are proud to share our experience and insight to help work toward the goal of lowering them industry-wide.
National Readmission Prevention Collaborative is a volunteer association created to unite industry leaders in sharing best practice case studies for hospitals, skilled nursing facilities and other post-acute providers. Through their advisory panels and available resources, their goal is to give providers the knowledge and tools to help reduce and ultimately prevent hospital readmissions for their patients and residents.
